A mis-pick is often a confident wrong identity. A 1-second visual confirm at pick time is cheaper than a return, a chargeback, and a customer who does not come back.
Mis-picks rarely look dramatic while they happen. The unit looks right enough. The bin was close enough. The label was almost the same SKU with a different pack count.
Visual confirmation is not a second pick. It is a cheap pause: show the expected product photo next to what is in the hand. If they do not match, stop.
Put the photo where the hand is
A monitor at the end of the aisle is too late. The phone they already use for lookup can show the canonical image beside the live camera. I keep this optional for trusted leads and default-on for new hires during the first weeks.
I log overrides. If one SKU eats overrides, it is a labelling or slotting problem, not a "people need more training" problem.
Do not add friction to easy picks
High-velocity, distinctive items can skip the extra glance. Lookalikes and high-cost items should not. Blanket friction is how operators invent bypasses.
